Sign Up Information Page

In order to sign up for counseling services with one of our licensed professionals please complete the Intake Questionnaire in order to provide us with the information we need to help you.

Once we receive your questionnaire one of your licensed therapists will contact you within 24 hours to schedule a Telephone or Live Chat session. If you requested email therapy, you should receive a response to your questionnaire in 24 to 48 hours.
